Israel is geographically too small to credibly present a MAD defense. They take one hit and they're done, just one. Fanatics in the Arab world know this, and don't care about the consequences, if any.

The Arab nations won't fire on Israel - for one, they would instantly be the target of pretty much every Western nation, and 2, they are so close that they themselves would suffer catastrophic fallout from such a strike.
