"Hot" Ratings

I've searched high and low and haven't been able to find an answer. How do stories get the "H" awarded? The "E" is obvious but I'm unsure about the H. Is it by votes or by the reviewers before the story is posted to the board?

I believe if your rating is above a certain number, you get the H, but not sure.
