If a chapter heading in a design spec is supposed to be initial caps & lower case then should the word 'orthopaedics' be capitalised (see line below)? This is a genuine question, please help...

"Diagnosis in Orthopaedics"

(this is part of a proofreading exercise I'm doing. Easily confused, you see.)

The last word in a head is always initial capped. Beyond this, a noun would always be capped, and beyond this "Orthopaedics" is long enough that it would always be capped in a head no matter what word form it was. (Chicago Manual of Style, 8.167)

So, three times yes.
