I believe that when a story reaches a score of 4.5 it is automatically flagged as Hot. I don't know the criteria or trigger for Hall of Fame status. Can anyone explain how that works?

AFAICT: for the "all-time" section, it's the 250 top-rated stories in their category, but restricted to stories with at least 100 votes.

For the "last 12 months" and "last 30 days" sections, the minimum number of votes is lower or maybe non-existent.

The red H's (Hot) denote stories which have received 10 or more votes from readers, and maintain an average vote of 4.5 out of 5 or better.

However I don't think those pages are updated instantly. So if you get 10 votes, and they average 4.5, but next morning it has dropped down to 4.4 (say, with another vote) you might never see the red "H".
