I think when in doubt, or even if not, 'my good fellow' is very appropriate. I would certainly have no objection to being addressed as such.

At one point I was going to insist on being addressed as Mister (which comes from Master)- as in I am the Master of my own destiny and so forth, but I realized that I was only inviting trouble and headache if I started checking the MR. box. so much confusion, lol.
