Typically the recipient is female, although not necessarily so. The person is bound and often times some sort of device is used - wand style vibrator, or Sybian for example. Males can get either be brought to orgasm 'the old fashioned way' or through the use of a pumping device that acts much as a milking machine. Diabolical.
Obviously the female has the advantage here as she can be brought to orgasm over and over again - whereas at some point the male will cease to produce ejaculate.
