I wouldnt say that you would always feel the man cumming inside; however most times you can feel the cock become harder. Lets face it, it is really hard for a man to hide his orgasm. I can tell but then I have been married to mine for some time now so we have become very in tune...

Sure, it's not realistic, but it works ok for some kind of super stud story or you're wanting to use a bit of dramatic license for better storytelling. If, for example you have a heroine that is known to have unnatural sexual ability, it might be reasonable. Of course, simultaneous climax is possible despite not actually being able to feel the cum spurting. I certainly don't buy it happening on the first time, though. I've managed to pull it off exactly once in my entire sex history. I actually sort of like the jet of "hot cum line" better than "they came together" even though it's not factually accurate.

I could possibly see the hymen thing, though, if it was some virgin that was turned on to the point she was practically begging to cum. Afterall, some women can get off on a direct clit whipping when they're really turned on, and I doubt it would hurt that much more.
