It's the same here for me Etoile. My wife says she usually does it palm down - also, she usually has her thumb folded under the four fingers, rather than a typical fist shape. To address your specific concern, I will say it is just about impossible for you to have a tear during fisting. Why? Because you're communicating with your partner! If you're feeling like something is stretching too much, mention that to your partner. They can check visually and also respond to your instructions. With a baby, that baby is coming like it or not, and that's why episiotomies are done. But the fist is never going to be biologically forced, so there's no risk of tearing.

Giving up hope.

Yep...its confirmed I have a narrow pelvis. From what info I could gather on the web, some women have this condition. Seems the ligaments in the pelvic area won't stretch nor will the bones ease up. I have heard that women who haven't given birth naturally have a problem being fisted.
