It seems possible for a woman to lie flat, face down, with her arm straight under her torso, and her fingers between her legs to pleasure herself. (Is the average arm too long for that?) Realistically speaking it's not really comfortable. You want to be able to move your hand and it's tough to do so when you are lying on top of your arm. As far as I know, breast size shouldn't matter here.

But you'd need to have a reason for a woman to pick that position over lying on their back.
