The same way it was handled in the case of George Clinton who was technically not eligible to be President at the time of his election and who was Vice President under both James Madison and Thomas Jefferson. I suspect that in such an emergency where the President couldn't serve the presidency would revert to the Speaker of the House if the Vice President wasn't a natural-born citizen.
