It is really bizarre and surreal looking...

The urethra could not possibly swell like that, it's not possible. You need muscle tissue for swelling to happen.
The urethra is a tube like membrane a bit like a vein but much thinner. It is only a couple millimeters wide.
It can be stretched to alarming proportions but it cannot swell.
What we're seeing is a prolapsed uterus/cervix unit and a very twisted woman.
I believe that she may not feel pain (or she's tuning it out) because when the pelvic floor and supporting ligaments give out and no longer hold the reproductive organs up, this can often result in nerve damage.
Also by the looks of it, she's had at least 4 or more kids and never did a Kegel squeeze in her life.

First of all, for her cervix to be this low and accessible means the uterus is prolapsed, the pelvic muscles aren't holding anything. That woman is already looking at nasty long term health problems right there.
