my 2 ideas are:

the guy visits his girlfriend, but he arrives earlier then expected. The door is not locked, so he just walks in, when he hears 2 people moaning. One is obviously his girlfriend. And she only moans like that during sex. He follows the sound and he ends up in front of the room of his girlfriend. He takes a short peek through the keyhole: There is his girlfriend, naked. She is kissing another naked person. The guy can't see the head of this person .... but he is very pissed - some guy is fucking his girlfriend ? No way, he will beat the hell out of this guy - .... so he rips the door open.
But then he is surprised. There is no other guy in the room. The other naked person (besides his gf) is the mother of the gf. She has lesbian sex with her own mother (by the way: even though her mother is 50, she still looks hot).
At first, everybody is kinda shocked and surprised, but finally, the guy looks at his gf and says "why didn't you invite me to the fun ?"
The gf doesn't know what to say, but suddenly her mom says "Well, you can still join us - do you think you can handle 2 hot women ?" (of course he can)

And the other one:

A guy is at home when he hears strange sounds from the room of his sister. He looks through the keyhole and is surprised: His sister is having sex with a girl ? She is a lesbian ? Not bad but my real thoughts did not involve incest or the peeper joining in. More like this:

Football practice has been cancelled. He pulls up to his house and sees his GFs car in the driveway. He thinks thats weird because she said she was going to the library but he was happy to see her. He walks into the house but doesn't see anybody. He figures they are out back so he walks to his room to change real quick. As he passes his mom and dads room he hears something so he peeks in. There he sees his mom (dad) naked sitting on the bed with his gf on her knees performing orally....

More detailed describtions are required but I think you get my drift. By the way he doesn't join in but does enjoy the show. In future editions he may watch both his mom and dad with her.
