Accidental Telepathy In A Story

I was recently rereading one of my older stories that I published as an ebook (it's not on Literotica), and I came across the following exchange:

“I guess your conscience is clear,” Maya remarked, her tone and attitude hardening again.

“That’s not the only thing that’s hardening,” Olaf quipped, his voice turning to a growl.

This is so silly that I wonder how I never noticed it while editing and proofreading this story. Notice that Maya doesn't say anything about something hardening (either in the excerpted quote or the entire preceeding exchange), it's just her tone and attitude that hardens; but Olaf somehow read my mind as I was writing and responded to my description rather than to what Maya actually said.

Has anyone else committed any fourth wall-breaking narrative errors like this?

This comes up a lot in role playing games. The game master has to say, “How did your character know what the players are talking about?”

Or sometimes a player has to call out the game master on it

You're in good company. Have you ever seen Ralph Bakshi's version of "The Lord of the Rings"? During the Council of Elrond, Boromir suddenly exclaims, "Enough of this talk of skulking and hiding!" It's a line taken directly from the book, but in the movie version no-one has mentioned it at all.

Poor editing happens. Sometimes the mistakes make you smile. Yours probably falls into that category.

Perhaps it was just a nightmare I had before I went to see Fellowship. Whatever it was, I was quite anxious that this unknown "Peter Jackson" had screwed it all up. That lasted about six seconds into the movie. "It began with the forging of the great rings..."

The Bakshi version finishes up with the Battle of Helm's Deep; I think the plan was to make a second part but that fell through. OTOH, the Rankin/Bass version starts with The Hobbit and then does Return of the King, leaving out the middle, so they kind of fit together.

On an aside, a little theater near the UT campus in Austin used to run double features of Gerald Potterton's 'Heavy Metal' and Ralph Bakshi's 'Wizard's' once a month. They were always packed and are both still on my all time favorites lists for movies.
