Absolutely. That's why most businesses have clauses that account for such things in their dealings with insurance companies.....I have to sign a waiver saying I don't smoke....if I smoke, I pay more.

As for the unvaccinated? Apart from possibly spreading Covid, they could be hospitalized and die, leaving more work to be done by those who have been vaccinated.
I'll defer. It's been a long time since I've had insurance. I used to pay for the insurance for a couple of dozen employees and don't remember any differentiation - but that was pre-Obamacare and I really don't remember.

The messaging would be confusing if every single doctor in the world didn't recommend the vaccination as a way to bring the pandemic to an end. Regardless of what you want the effectiveness to be, it does contribute to reducing the infection rate, which is the direct metric for the pandemic.
