It appears to me (....and correct me if I'm wrong, people!), that it is a thin tube taped to a vacuum cleaner hose. The thinner tube has a sharp pin attached to the orange plug - which I would assume is hollow to allow air to be sucked past it!

With the vacuum cleaner switched on it would suck the nipple or clit up into the thinner tube and onto the sharp pin!
