I did one like this, although it was historical-erotica.
My story was set back in the 1930's when a lighthousekeeper had a shipwreck where only the coal trimmer survived the steamship wreck on the reef. He made it to the lighthouse before collapsing from the cold, and woke up revived with the lighthousekeeper wife marveling over the size of his shaft, and stroking it to release so he could get a good nights rest.

To make it edgy, the guy was 15 when whacked in the head and 18.5 when awakened from his coma by one or more hot.nurses (any genders) stroking his shaft. So he's physically adult but mentally adolescent, and overwhelmed by this first sexual experience (that he knows of). Make it clear that nursies didn't start handling him till after his 18th birthday, when he seemed to need the treatment for some reason the author can invent.

That is an interesting take, hmmh
But that's 3 years in a coma, that's a lot. I'm concerned about the realistic detrimental effects that would have on the body, atrophy of the muscles etc, that's not sexy.
