As I understand it, the only additional qualifying factor in Obamacare is whether or not you smoke. I think that's fair and also think the government should not be allowed any other latitude in letting additional "pre-existing conditions" be determinants.

This is especially important in the age of Covid. Now, to answer the first question, no one lives the healthiest lifestyle all the time. We've all got our vices and bad habits. That's already worked into health insurance policies (i.e. smokers pay more) and I'm fine with that.

No it shouldn't...all civilized Countries should be working towards Universal Healthcare, seeing it's citizens as valuable human beings, not just as a labor force.

Here in the USA we are not civilized yet, we allow for millions of people to go without access to healthcare which has the net affect of driving up the cost of healthcare for all those actually paying into the system. It also puts the entire country at risk for collapse...as we are seeing right now with the covid pandemic in the USA.
