First of all, you need a female character to be more kinky and outspoken about her fantasies. You can’t get a situation where three shy women having sex with each other’s husbands. Make at least one female character little bit slutty. She can lead the other two into the situation.

I like your idea. There are many situations which can create the orgy. One particular and most popular situation is to have a home party involving all the characters. Make them drunk. One of them (mostly the slutty wife) crosses the line. Others watch her and act according to their fantasies.
