Being polite or a gentleman is a learned response. Most fellows learn it at home from their parents. Even when I first started teaching, I made the kindergarten boys hold the door. They were taught to 'help' when someone had a heavy load to carry. I was allowed to take my classes on field trip using public transportation. Just me and my 30+ kids. The boys were told to give up their seats if an adult needed one. Only when all the boys were standing did the girls give up theirs. All of my classes were taught manners! 99.99% of them were taught manners at home, so I was just reinforcing them.

I would think that even men who have handicaps would make an effort to help you, if only to close the car door.
