She thinks her boobs are too small to attract a man. The stepfather forbids it. She shows him her boobs; they are rather small, but still nice looking. Of course, for a Lit story, I would remove the "step" relationship and make them biological relatives.* Also maybe make it more of a deliberate scheme on the part of the daughter than mostly the father's self-delusion. But there is lots of room for variation.

Does he joke about her flatness? Does she turn red or does she joke back? Does Maddy casually monitor daddy's pr0n-viewing? Caught, does she gently lead him to the shower?

I'd use a standard distant-daddy setup. Maddy has been in an all-girls academy even before mommy died / divorced / disappeared / drank-down / whatever. Quite right. I see abuse by teachers, pastors, any close authority figures, as being equivalent. If someone in power compels sex with you, it's rape.

And even bans of step-sex may be inadequate. If the adults marry, they become step-parents to the others' children, who then have legal protection in some states. But a common-law hookup, shacking-up for a few months or years, exposes the unrelated kids to an adult's authority and abuse without legal cover.

In LIT fantasyland we separate compelled from consensual incest. Compelled is ugly. Consensual is... taboo, and exciting, and eventually OK, at least as stroker material. And for strokers, step-sex ain't really incest, ho-hum. It's only kinky enough with shared DNA. Gotta please the audience...
