Not everyone can. Honestly, I think that out of the blue "cum now" (without any prior stimulation, etc.) is mostly a work of fiction.

Not an expert but I am thinking people respond to different stimuli in different ways. For example, clit action will drive me over the edge pretty much every time, but penetration alone does not seem to do it. Whether that is hard wired or is just a matter of practice , yet to figure out.
