I do as you do, I ask first. If my audience finds it offensive, I avoid it. In speech, I usually reserve it for women who cut me off on the interstate or who fail to yield when I have the right of way. In writing, I prefer the diminutive - cunny - or quim, but I'll use cunt when I need a strong, possibly offensive Anglo-Saxonism.
