DVS - I know from how my husband and I play with breath play, that there is a certain part of my neck that he will press down on that is the safe way to do it. He's given me the low down on how not to do it. Several times when I've tried to choke him out and have had my hands positioned wrong. And with our play as soon as I'm out, he lets go. 600 people seems like a lot, but it's worth remembering that many more are killed by stupider things.
I like breath play, breath retention and so on, but I've never gone to the point of putting someone under. However intense it might be, for me it's not worth the risk.
