That's one of my favorite sexual positions, but not everybody can do it, and knees under the armpits is definitely optional - it's really a variation on 69, with the man on top, although the feet thing is an interesting twist. In a more traditional 69 position one runs the risk of getting ones skull crushed between her thighs, which is not all that comfortable. With the pussy staring you in the face like that, you can hunt and peck, and with the thighs out of the way, your hands are free to apply, er... additional stimulus.

You can usually pin her legs back under your armpits: most women can handle that for a while unless they have hip or back issues, and it's actually a good flexability routine for the lower back, just approach it like any stretching exercise and don't overdo it and tear a muscle, and your flexibility will gradually increase.
