but how do you build a story around this fascination/fetish?
 
You have MC obsessing over someone's shoulders. Person is unobtainable for some reason so MC can only describe the shoulders in various ways and imagine what they would like to do with them. Then something happens to spark hope for the MC. 'Could I ask a favor? My shoulders are killing me.' Person is still unobtainable but there is an opportunity for risky, one-sided flirtation. 'Oh, did you need your shoulders rubbed again?'

But mainly, lots of varied descriptions and how the shoulders make MC feel.
 
As a student I was often in a form of amphitheatre. Rows upon rows, each ine higher than the last. The next row is close below you.

It is summer. The sleeves become short. Spaghetti tops and sleeveless shirts galore for the woman (male perspective because I am one. I'll not pretend to know the female view). The backpack or shoulder strap bag was heavy. Red stings on the skin, and they roll their shoulders often to get them loose. You can see it up close.

Does it really hurt? The protagonist is good at massages. Maybe she wants one?

They get to talking, where he's non creepily complimenting her shoulders. Appreciating how they feel, how they respond to him. She starts to take notice he really likes her shoulders, but the massage is too good to think about if she should let him be horny with her shoulders.
 
I do find shoulders fascinating. If I've got three or four women within my sight, and only one has bare shoulders, that's the one I'm going to be looking at.

Some women love to bare their shoulders, but others are afraid to. They think they are too bony, or too flabby, or whatever.

The backrub or massage idea is a good starting point.

The phrase "a shoulder to cry on" might be a good starting point for a story.

The novel "Atlas Shrugged" was originally going to be called "Alice Shrugged" and was about a woman who liked to flex her shoulders to attract men.*

Or more realistic: "I think he's cheating on me." <Cries on shoulder, then kisses>
 
When he first sees her, she is behind some kind of obstruction. All he sees is her beautiful face, hair, and shoulders, and seeing no sign of apparel, can not but imagine she is out there, in public, in the nude. Of course she is not, she is in a seductive offshoulder dress or top. But even after comfirming that, that first mental image does not leave his mind. That beautiful come hither smile, framed by the most delicate nude shoulders he has ever seen…
