As I posted before, the states are gaming the system too. They lower the threshold to qualify for medicaid so as to get bigger checks from the feds. In my state the multiplier is 3x, in other words for every dollar the state spends it is matched with three dollars from the feds (different states have different multipliers). The game here is to tax the hospitals and doctors on a different schedule and to then give the money back in the form of medicaid payments, that's a 300% ROI.
