Mother is doing laundry and finds a pair of her panties in her college aged son's basket.

She smiles and muses to herself ... 'what a filthy little boy he is.'
 
I actually like the 'little'. That feel's real for the mother of a college aged kid.

I might preface the sentence with her carping to herself "he is twenty, why am I doing his laundry?" Also realistic and it emphasizes the actual age in close proximity to the risky phrase.

Yes, add a note to the editor, drawing attention to the context and usage. Or write a more imaginative story.
 
It's hard to tell if it won't trip the filters, but it's definitely fine rule-wise if the 'little boy' is established to be of age. (Some say college is not enough because there are edge cases).

I'd just submit it as-is, and if the story gets bounced then you drop a note to admin that the guy has been clearly said to be 20yo or whatnot.

Dropping the note with the initial submission would likely prevent the "bounce" to begin with.

As long as you already established that the son is college aged, that should be no problem.

I'd work in a reference to his age before you get to that line, to make it clear.
