You are talking about runway fashion. It isn't meant to be worn by regular people in public, very few pieces are made and end up bought by uber rich/trendy people and customized for specialized events. It is meant to look provocative.

For the runway collections you see, the designer also creates a ready-to-wear collection that is down to earth and actually bought by upper income people.
