Two names?

Is it just me, or has anyone else noticed in a few stories the names will be different than when they started out? Why is this.

Here's one I just read. It was a great story, but the name thing just confused me a bit.

I've had a few authors write me the day their story is posted saying, "Oops! I changed the protagonist's name, but I missed a couple spots." It happens...if you see a story like that, it'd be super cool if you could write the author a quick email letting them know. I try to catch em when I can, but my eyes aren't as sharp as they once were...
