It all depends on the position of the couple, and how restricted the movements of the woman are. The humilaition element has so much to do with the attitudes of the two people. If he's being a bit stern and rough, showing little regard for her enjoyment and comfort, then that's what humiliation is all about. So much of it is in the mind, and the way the woman is made to feel.

I would think it would be humiliating under certain circumstances. If the man forces himself on her and she doesn't want it, then yes but you could say the same about anything. I have never done this but I have seen it in movies where the woman initiates it as part of foreplay and I have included it in some stories, either as a final sex act with the man ejaculating or as part of a woman performing orally. Anytime it was ever included it was at the woman's suggestion or initiative.

An equivalent to a female orgasm would probably be a man using his finger(s) either to frig or to penetrate. That would not be exact but it is probably as close as any other kind of contact.
