The answer to all 3 of these questions is: it depends.

1) It can be done on purpose. But most of the times women don't tend to try and flash their pussy to any stranger that might see it, so it's 99% accidental.
It can happen from poor choice of clothes or underwear or both.

1) As far I know women, when it comes to looks in public very little is left to accidence, and they usually know damn well what effects those looks have even if they pretend innocence and ignorance. So, it may be misfortune in few cases, but even then rather planned misfortune.
