You seem to want hik to be driven by sexual desires. Priapism isn't necessarily good for that. I would go one of two ways. Unfortunate biology, where he's got a rare condition where erections happen near constantly. Like an uncontrollable 18+ puberty. The other is mentally. If people can get addicted to sexual stimuli so much so they continue if their genitals start to bleed, a form of addiction to sexual stimuli is not far fetched.

As for the story I would go the desperation route. A strong emotion causes him to try to drown his sorrows. His GF left him, someone close to him died, or an obsession to be validated in the sexual field, but everything feels hollow until he finds something or someone.
