I like the idea of an older woman, such as a principal or English professor, having a beauty wart on her August ass which she views as a sort of personal signature. Any photo of her ass with the wart in it confirms it's her, and she uses it as a form of humiliation by having subs kiss and caress it, perhaps leaving lipstick marks around it. On the other hand it can be used against her in the event her ass is photographed in compromising situations. Paying tribute to her wart is seen as a natural introduction to her in sexual situations. The privilege of meeting the wart could be a highly sought after item.
 
I think the mole would be a perfect lead in for an ass worship/ anal focused story. The 5 W's are completely open and the wilder the better.
A MILF trans-woman meets two young adventurous couples at Fantasy Fest. Bi-men aren't my usual interest but it would fit well into this story.
Some mommy role play? Sub/Dom?

I like this idea a lot and I'm going to write this one down in my notes if you don't mind.
 
I like the idea that she is of old WASP stock and comes from a long line descended from the original Puritans, with lots of relatives in the CIA and MI6. She has a cold, regal air that effortlessly commands deference from others.
 
Thinking as a reader, I would rather see the word "mole" or "birthmark" as opposed to "wart" which I'm pretty sure would turn my erection into a puddle.

A birthmark could tie into your concept of a long family bloodline, perhaps all sharing a birthmark on their butts.
 
Sounds like my type of story, though like @dirk2024 said, the words "mole" or "birthmark" are a better fit. Unlike @Rob_Royale 's take, mine would feature all women, even with the protagonist being trans.

Moles are, however, pretty much consistent. Birthmarks on the other hand can have different shapes. If I was writing this story, I'd give the birthmark an unique shape, like the shape of a heart, for example.

since they have different shapes why restrict yourself to something basic, like a heart? you can ask for anything your heart desires.

Because I mentioned a heart as an example. If you want to write intrinsically designed birthmarks that would compete with the most complex Buddhist, Hindu, or Thai pieces of art, you're more than welcome to try. Your writing is valid, write whatever you want.
 
I like the birth mark idea also, a wart is not sexy a mole maybe but a small tattoo or branding mark from a previous life maybe?
