We all know that it's some sort of author felony to end a sentence with a preposition, but what's the case for having a quote begin the first line of a story? Is that a major issue?

You mean quote as in a quotation? Or quote as in "Hello," he said?

I assume he means quotation.

As the first line of text, probably not. As a sort of sub-title below the (chapter) title and before the text is fine.

An example might be:

What Katy Did

To bed, to bed, yelled Sleepyhead.

What Katy and her friends did was shocking. ....

We should never end a sentence with a preposition? That is nonsense up with which we should not put!
