It's an interesting idea. I'd start it off where she hires an escort to have the actual sex but she's in the room taking notes, offering advice. Husband watches from the door and is wildly turned on. The escort makes a sexy remark on how much the patient would enjoy two women in the bed and the therapist just dismisses it. But the husband can't help but fantasize about it. He mentions it passing to make a cute remark and the wife starts to think about it.

No need to go full cuckold, but a series of fun threesomes would be hot. A FMF with the escort and then a MFM with the patient.

Not saying you can't write it this way but in reality, escorts, sex surrogates and sex therapists are three entirely separate occupations and no one person would be carrying out all three roles. Therapists and surrogates work in tandem with each other and plan out the patient's treatment plan well ahead of time. Contracting an escort to fulfill the surrogacy part of the therapy would not happen for a variety of ethical and practical reasons. Also the husband watching any part of the therapy process is a violation of privacy guidelines.

As I mentioned above, sex surrogates wouldn't just "plan" things on their own, any treatment decisions for the patient would be made with the sex therapist prior to the actual session.

I've actually thought about writing a story related to sex surrogates because it's an interesting situation to think about, but there are several factors to consider if you want to write something believeable. But, as this is fantasy, please feel free to disregard everything I've mentioned and write the stories how you like! I just wanted to provide a little bit of additional context.
