From what I have seen, read, and know, it is a practice seen in the extreme side of BDSM. Though it is a recognised form of female genital mutilation in some cultures, I do not know if it qualifies as that in the lifestyle as it is not usually permanent, more a scene thing, which under the correct conditions with appropriate precautions, would not cause any permanent damage. Certainly has it's fascination factors.
