The idea is to inflate them right? The effects last something like 48 hours, but if it was only a little saline more like 24. I've never done it, just read about it.

Supposedly, although the IV to get the saline in there hurts, the swelling itself doesn't. Ballsacs are apparently ridiculously stretchy.

Assuming you are careful, using sterile solution and new needles and know *what* you are doing so that you're not injecting into the bloodstream in any way, you should have no more problems than another kind of needleplay scene.

Please read a LOT before doing this, remember I said "knowing what you are doing" - that can't be overstated.
