We don't know yet until there is independent analysis.

4. Was the Assad regime responsible?

We don't know. Probably they were for the attack on the hospital but we don't know.

We don't know. It might have been the opponents' act to attract condemnation of Assad's regime. What we do know is that many factions in Syria's war, not just Assad's regime, have committed frequent war crimes.
