Question for Adult Babies/Ageplay-ers

I am a horror/erotica author specializing in noncon/transformation stories (forced fem, forced chastity, etc). For a fun spin I am writing a story about "forced adult baby-ism" aka a victim is essentially reduced to an infantile state. For how to do this I've introduced malnutrition (only being fed baby food) to reduce body size and make bones/body/muscles really wasted and brittle. I've also had my villains extract the victim's teeth so he can only gum stuff/has to "teethe" on ice packs while they heal. What are some other ideas? It's a horror/erotica crossover, heavy on the horror element, so nothing is too weird for consideration.

To help the weight loss, wouldn't a steady diet of laxitives help drain the body? Sure would make the diaper changes a bit more interesting. Regaining fluids by breastfeeding from a 500 pound lactating lady would help, and the 'horror' part of it could be that he can only feed as she lays on top of him, making him pass out in less than a minute. This could be from the pressure on his chest, or the 44GGG breasts suffocating him (nap time!).
