Expected publication will be sometime in the late 2005 time frame. Please write back to us to let us know that you are interested in accepting our offer and we will send you the publication contract.

As part of the publication process, we need you to email us the following:

--Previously Published Information: If your story has been previously published, please send the exact information of where and when it was published, as this must appear in the copyright page of the book.

--Author Cover Bio: Usually about 100 words or less. A short, one-paragraph description of yourself that will appear in the book. Please write this in third-person perspective.

--Author Biography: Can be to 1,000 words. This is a longer description of yourself or a message to your readers. It appears on the author biography page that readers view when they click on your name on the Phaze website. Please write this in third-person perspective.

--Author's Photo: We need a nice quality (large size is OK) high-resolution color digital photo of you. This should be at least 600x900 pixels if possible, high quality JPG image preferred. This will be used in the book itself, as well as on your author's page and other places on the Phaze website and Phaze vendor sites.

--Quotes and/or Reviews: If you have received reviews for your story, please send us all of that information.

--Awards: If you have received any awards, or been in the running for awards, please send us that information.

--Up-to-date personal information. All personal information we receive from you is held in the strictest of confidence and will only be used for our business relationship as required.
We need the following information:
--Your current e-mail address.
--Your real name.
--Your pen name if you want to use one.
--Your snail mail address (no post office box as books shipped to you come UPS).
--Your current phone number (home/evening preferably).
--Should royalties exceed $600.00 per year we will need your social security number for the 1099 form that is issued for tax purposes. Do not send your social security number at this time.
--Also please keep us updated anytime any of this information changes.
