I usually start with a specific scene or situation in mind. Most of my stories aren't very complicated, and a lot are just sex scenes. I'll have the scene in mind, and then I'll figure out what I have to do to get my characters there and start writing. In the course of telling the story the characters come alive or I notice something and the story takes on a certain subtlety or depth, but generally my plots are very simple.
I do most of my thinking while I'm writing. Sometimes I walk around the neighborhood telling the story to myself, but most of the time my insights and ideas come while I'm doing the actual writing.

Doc M said that but I could have said the same thing. Sometimes I'll see a sexy pic of a woman, sometimes porn and sometimes not and that will inspire me to write about her. Sometimes an idea comes to me out of the blue. I wrote one incest story, "My Cute Nieces", after seeing two of my cute nieces.
If you are turned on by women's feet, that would make a good subject to start. Write your first about that, and then find something else for your second. Or, write about feet again.
This is really a very supportive group but there are certain rules to learn, and you will learn them as you go along.
