I won't pretend to speak for "men" in general, but I CAN tell you how it is for me. I spent a good part of my life denying that looks are important, and pretending that I didn't care how I looked. Much of the time I was so successful at it that I didn't consciously realize that I was pretending. As a result, I didn't acknowledge the stress that maintaining the fiction was putting on me. Now that I have given up trying to believe that nonsense, I spend more time doing something about my appearance, but the overall result is considerably less stress.
