The saying has survived and thrived for nearly a thousand years, and perhaps longer, although it is difficult to properly attribute phrases of such antiquity.

It has ever meant, "speaking poorly of someone out of their presence" and its earliest roots appear to be biblical (meaning of course (according to the currently running anti-christian/pro-christian thread) that it is wrong and stupid).
