I was thinking of incest set in a parallel Earth which is connected to Earth by the Bermuda Triangle. It would be in the context of a longer, broader story of a world still under feudalism, not because its people were inferior or anything, just because there hasn't been an Industrial Revolution yet and feudal lords have become sovereign, defeating the monarchs and forming their own countries. There are some 17 feudal states on this world, out of what used to be 8 kingdoms on this world with a single continent.
The incest component is this: Sophia, last Countess of the old ruling house in Gavinium, a free County, inherits the throne from her elderly brother, a former monk and lifelong celibate. She has to fight a civil war to do so. During that civil war, in which she is briefly ousted, her rival rapes a married woman. Her husband gets revenge by helping Sophia capture the usurper and regain the throne.
Realizing that it is too late for her to have children at the age of 53, she issues the Grand Edict. It requires her successor (whom she adopts in her will to avoid future strife) to take all of the available women of his family to his bed. In the future, progeny will be found and lusts will be satisfied primarily by inheriting relatives of both sexes as spouses. If a man inherits, as is the usual case, he gets to enjoy his mother, sisters, aunts, etc., as well as his brothers, uncles. If a woman inherits, it's similar. The relatives can only fuck each other, and with the ruler's permission. No outsiders are permitted them.
I will incorporate Ms. Linnet's idea, just for kicks (but modified to make it gender-neutral) by making infidelity treason, a capital offense (quite realistic in such a medieval culture). It's only a partial double standard, since they can still fuck each other, just not outsiders. The ruler can fuck whomever he or she pleases, though there is less need to do so with such a large harem.
75 years later, the new Count Alec finds the idea of fucking his own mother repulsive, but he is faced with his duty to make her his first wife, as is custom. His previous sex life had been endurable, since he only had to take an occasional cock up the ass from his dad and fuck his sisters now and then.
