On more then one occasion I have become very lighted headed after some very intense sex. Been there, done that! On several occasions, in fact.
In addition to blood pressure changes, I can think of two other possible reasons for passing out. First, I've sometimes found myself hyperventilating during sex; eventually the buildup of CO2 could cause unconsciousness. Second, on the opposite end of the spectrum, I'll be so turned on and so focused on the sex that I'll forget to breathe! That'll knock you out for sure!
While neither I nor my partner have ever blacked out during sex, we've both entered altered states of consciousness, sometimes it seems like an out of body experience. (I've always wondered if I was on the edge of nirvana; according to buddhist beliefs, sex is one of the paths to get there.)

Some time back I read this is not an uncommon thing. There was an explination, but I do not remember much about it now-something about the blood flow I think. The headaches are not that bad, and do not last very long, an hour or so. Only time I have experienced them is after an orgasm, not from lack of orgasm. Better to have them after then before.
