Each district sends one male and one female (over 18 yo) to compete in the annual Hunger Sex Games which will be broadcast to the country. Contestants are fitted with an electronic monitoring device that measures sexual arousal and orgasms. They are put into a sexual arena with the only rules being that the last person to orgasm is declared the winner. Contestants team up and work together to eliminate competitors. Many options available (MF, MMF, FFM, FF, MM) maybe even a contestant or two becoming so aroused watching some of the others that the masturbate and eliminate themselves, or perhaps a twist at the end with two siblings working their way to the final two and then going at it to determine the ultimate winner........Jealous bf and gf back home can watch all the action, etc

My qualm would be the death aspect. I love sex but Im pretty sure Id castrate myself if thats my only means of survival. (think wolf chewing through own arm)

I assumed it wasn't necessarily the last one to make *themselves* orgasm. It seems like a nonconsent/reluctance story to me. I assume that the punishment for orgasming wouldn't be death, just ejection from the tournament.

Not sure what sort of prize you'd have, though, or what sort of setting you'd create to justify everything.
