How to combine separately posted chapters into a "series"?

I am about to post a 5 chapter story, and I want to know how I can keep the chapters together as a series.

When submitting multiple chapters with the same story name, the moderators have automatically grouped them into a series like that for me. I expect that as long as you have the same name for each submission (i.e. Test - Ch. 01, Test - Ch. 02), they will appear under a single 'Test' heading on your profile page.
