I hate to say it,,BUT its been a few years since I have last had a passport
i need birth certificate
ID
forms from the issuing office
anything else?
someone said i had to show proof of departure,,,that may be a problem as i am a bit unsure of departure date
anyone know if I HAVE to have plane ticket first?

You need a picture

You don't have to show proof of departure or your plane ticket unless you're going to a "quickie" agency. If you are waiting the 6-8 weeks to receive it in snail mail you only need your birth certificate and ID.

And make sure your birth certificate is the original and not a copy.....they won't take it....

If you have a ritz camera location or a photography center they can get you passport photo's....
