Pretty straightforward concept. Guy's car breaks down in rural area and the only house for miles is owned by twin sisters who commonly finish each other sentences, carry around knives and cleavers with blood on them on midnight walks, and seem closer than twins should be. They say that the man is welcome to stay but the room he'll be sleeping in doesn't have a lock and it belonged to their father, who passed away under mysterious circumstances years ago.

Guy's reaction? A smile, a nod and growing bulge in his pants. The guy loves how odd the twins are and intends to bed them both before he leaves.
 
And, so, does he fuck them? Do they murder him? Does dad's ghost fuck him or murder him? Are they cannibals like in rhe Wrong Turn movies? Does he live happily ever after? Do the twins fuck each other?
 
I figured they do have sex, with all three getting involved with each other. As their basking in the afterglow, two things can happen:

1. The twins explain their odd behavior. Turns out that they're fairly normal just eccentric.

2. Turns out they are creepy, possibly supernatural, killers they like him though and want him to stay. Our lead politely runs for his life
 
I think I saw that scenario a zillion times in EERIE and CREEPY. Sometimes the guy is an escaped axe-murderer or sheriff's deputy but he stands no chance against the sisters. But more likely he's an innocent traveler. Sometimes their remote aerie is a way-station in his past. Sometimes he seems lost but he's really their guardian angel, or they are his.

But usually they're witches or spiders etc and he's doomed. The pulps usually didn't go into fuckfests but I'm sure a few could be worked in here.
 
The main thing I want is for the guy to be exposed to the weirdness and be unfazed by it until something malevolent actually is confirmed or happens in front of him. For example, he could go into one of their rooms looking for something, and come across a book on human sacrifice or a doll that has his photo taped to it and he'll either think it's neat or be distracted by something else ("is this a pear of anguish? Whoa, is that their panties?")

Another setting I thought was a guy has to change school for the end of his senior year. He notices two twins are in his class that no one talks to. He decides to try and get to know them, even after he's warned they're witches. He manages to get on speaking terms with them and asks them out on a date. The sisters are amused and agree. Aside from the lights breaking everywhere they go, a swarm of birds attacking some people, and a dog with a human face showing up to give a bone-chilling laugh, the date goes great. He then asks them to prom and they both agree, whispering how he'll be perfect for their plans. He just smiles and is excited, especially when they give the condition that he has to come back to their house afterwards for something special.
 
I like the idea that Hypoxia hinted at...

The guy is a total psychopath and has murdered several beautiful women out of sheer hatred for women in general, and the way he has been treated by women his entire life. HE is on the run and the authorities are hot on his trail when he breaks down.

When he goes to the house he plans to kill the occupants and steal whatever he needs to continue on his way and escape from justice. When he meets the two weird twins he doesn't feel his usual hatred, but instead is immensely turned on.

The tables are turned and the twins have their way with the man, making him pleasure them in all sorts of sick and unusual ways. (I'm thinking watersports at a minmum)

The man wants to kill the twins and make his escape, but he cannot make himself hate them enough to actually kill them. In the end the man is no match for the obviously psychopathic twins, and they kill him just before the detective that had been hunting the man arrives at the house.

Leave it up in the air as to the peril that the detective is now in.
