Thought about this one earlier and thought it might make a very interesting twist ending.

He sits in the doctor’s office waiting for the doctor when he shows up, his imaginary friend. All his life she has always showed up when he was sad, scared, or just lonely. Except to him she is not imaginary. He can see her, hear her, and even touch her. She comes up behind him and rubs his shoulders trying to comfort him. After years of hiding it he went to the doctor and a cat scan revealed abnormal growth in his brain. He is worried he might have cancer. She reaches down and pulls out his cock and begins to stroke it as he waits, she walks around and begins to give him a blow job until he blows his load. If feels so real to him he thinks as she strokes him faster and faster until he cums. He snaps to as the doctor comes in. His cock is still in his pants and he had imagined her pulling it out. But he can very much feel the cum in his underpants.

The doctor comes in and tells him that he doesn’t have cancer and the abnormality is a growth of perfectly healty brain cells but a DNA test revealed that the cells or female. The doctor begins to tell him about Chemerism. He should have been born with a twin sister but there cells fused during the early stage of pregnancy when they were just a cluster of cells. A portion of his brain is made up of cells from his twin sister who was never born. He can see her, hear her, and touch her because she has apparently has some level of consciousness. The doctor tells him that their brains or two intermingled to be able to separate them.
 
What a twist!

Actually I think this is a really great idea, but might work better drawn out over several scenes that heighten the tension and his concerns over what's happening to him. Explore his reluctance to reveal the truth, his discussions with her. I'd also turn the ending around and say that they can and should operate on him, giving him the choice (he chooses not, of course).
 
There is a Stephen King story based on the same concept - The Dark Half. Only that one was more about murder and horror than about sex. I bring this up not to say your idea isn't original, but to show how good it is. Go for it!
 
I dyslexiated the thread title as GUY'S IMAGINARY FRIEND IS REALLY A HAMSTER. Kinda kinky.

Anyway, take it one step further. Triplets. Two feti inside the protagonists. Two voices within, always arguing. Devii or angel? Hilarity ensues.
 
I think this is an excellent idea. It would make a great short novel. You could really get to know the "imaginary" friend. They could have all sorts of episodes - she gives him advice, she gets jealous of his girlfriend, he gets her to try kinky stuff the girlfriend won't do. All the while the guy sees psychiatrists, neurologists, exorcists until finally he is diagnosed with visual and auditory hallucinations induced by a tumor in his brain. They do a biopsy and - bam! - it's naaht a tumuh, as Schwarzenegger would say. I also like the idea of the guy having the option to have the absorbed twin surgically removed but deciding not to.
